When a permanent tooth is knocked out, the clock starts immediately. The cells on the root surface that allow the tooth to reattach begin to die within about an hour outside the socket. Handling the tooth by the crown rather than the root preserves them, as does keeping it in milk or, best of all, placing it straight back into the socket and biting gently on gauze.
